Northstar Health Systems is a growing Community Supports provider offering short-term housing and support services for individuals recovering after hospital discharge. We work closely with healthcare partners to provide a safe, stable environment while clients transition to long-term housing.
Role Description
Northstar Health Systems is seeking a part-time Case Manager for an on-site position in San Bernardino County. The Case Manager will assess individual client needs, develop personalized care plans and deliver case management services to promote comprehensive wellness and support. Key responsibilities include coordinating care with healthcare providers and social service agencies, monitoring progress (writing progress notes), advocating for client needs, and providing consistent emotional and educational support. The role requires ensuring that services align with organizational standards and compliance requirements.
Qualifications
Strong skills in Social Work to provide empathetic and effective support to clients.
Proficiency in Case Management, with the ability to develop and monitor individualized care plans.
Experience with Mental Health services and an understanding of resources to support clients facing emotional or psychological challenges.
Exceptional organizational and communication skills, along with the ability to collaborate with healthcare providers and community organizations.
Bachelor’s degree in Social Work, Psychology, or a related field (Master’s degree preferred).
Current state licensure or ability to obtain licensure as required.
Familiarity with compliance and ethical guidelines in a healthcare or social service setting is highly beneficial.
Ability to work effectively in an on-site.
