Job Summary
Unified Care Hospice is seeking a compassionate and dependable
Bilingual Certified Nursing Assistant (CNA)
to join our growing clinical team. In this role, you will provide hands‑on personal care and emotional support to patients and families in their homes and care facilities. Positions are available for
Full-Time, Part-Time, and PRN (as‑needed)
schedules.
Bilingual (English/Spanish) candidates are strongly preferred to better serve our diverse community.
Employment Status Options
Full-Time: Regularly scheduled hours with benefits eligibility
Part-Time: Consistent, limited hours based on availability and census
PRN (As Needed): Flexible scheduling based on patient needs; no guaranteed hours
Responsibilities
Provide personal care including bathing, grooming, dressing, toileting, and feeding
Support patient comfort, safety, and dignity at all times
Monitor and document patient condition and vital signs in the EMR
Follow the individualized Plan of Care under RN supervision
Assist with safe transfers and mobility using appropriate equipment
Maintain infection control and safety standards
Communicate clearly with patients, families, nurses, and leadership
Complete daily/same‑day charting per agency policy
Deliver supplies and assist with basic patient support as needed
Participate in required trainings and in‑service education
Qualifications & Experience
Active CNA certification in the State of Texas
Current CPR certification
Bilingual (English/Spanish) preferred
Valid driver’s license, reliable transportation, and active auto insurance
Prior hospice, home health, or long‑term care experience preferred
Familiarity with EMR systems
Strong professionalism, reliability, and time management skills
Ability to work independently and within an interdisciplinary team
Experience with dementia, behavioral health, or end‑of‑life care is a plus
Physical Requirements
Ability to lift, reposition, and assist patients safely
Ability to stand, walk, bend, and stoop for extended periods
Ability to travel to patient homes and facilities
