Role Description
The purpose of a Clinical Informatics Specialist (CIS or Medical Scribe) is to support primary care providers with clinical documentation so they can focus on patient care. Scribes assist throughout the patient care journey – planning in morning huddles, observing and documenting patient visits, and coordinating post‑visit follow‑up. They become experts in value‑based care, chronic condition documentation, ICD‑10 and CPT coding, and help close care gaps. Scribes receive extensive on‑the‑job training in workflows, preventative care, and population health data. Candidates should commit at least 1–2 years to this role.
Responsibilities
Documenting Patient Encounters (≈ 80%)
Join provider in the exam room to observe visits
Document encounters in structured notes, including history, assessment, plan, and exam
Assign CPT and ICD‑10 codes
Prepare after‑visit summaries
Consult with provider to ensure accurate documentation
Clinical Documentation Improvement (≈ 10%)
Request and review medical records
Use population health tools to support documentation improvement
Prepare for and support daily huddles and documentation reviews
Consult on documentation opportunities
Administrative support (≈ 10%)
Place orders and referrals
Address tasks
Support the care team with additional documentation responsibilities
Other duties as assigned
What We're Looking For
Knowledge
Medical terminology and common medications (required); either from a pre‑medical degree or prior clinical experience
Prior clinical experience (shadowing/volunteering) – strongly preferred
Prior scribe or transcription experience – preferred but not required
Skills
Advanced listening and communication skills (required)
Strong computer literacy and adaptability to new workflows (required)
Fluency in Spanish, Polish, Russian, or other relevant languages (required where indicated)
Abilities
Adapt to new workflows and learn quickly (required)
Typing ≥ 70 words per minute – strongly preferred
Follow direction and work as part of a patient‑care team (required)
Self‑starter within role scope
Excellent attendance and in‑person availability in clinics (required)
Commit to at least 1 year in the role (required)
Work approximately 40–45 hours per week during clinic hours (required)
Comply with hospital and Oak Street Health policies, including HIPAA (required)
US work authorization (required)

$17.00 – $31.30 per hour
