Provides case management for patients and families to enhance their psychosocial functioning. Performs duties and responsibilities in a manner consistent with mission, values and Mercy Service Standards.
Position Details
Education: Master’s degree required in Social Work, Counseling or other human service-related field.
Experience: Minimum one-year Behavioral Health experience preferred.
Certifications
Other: Preferred Licensure: Licensed Master’s Social Worker (LMSW), Provisional Licensed Professional Counselor (PLPC) or eligibility for licensure preferred.
Preferred Certifications
Co-worker must be able to communicate orally and in writing and demonstrate proficient computer skills. Co-worker must be able to listen, understand and explain complex situations.
