
Licensed Practical Nurse

Become a part of our caring community - Conviva, a subsidiary of Humana, is looking for a compassionate and cheerful LPN/LVN (Licensed Practice Nurse / Licensed Vocational Nurse) to provide professional nursing care to patients within an assigned unit, clinic or region.
Location: You will work on-site at the Conviva De Leon Care Center and report directly to the Area Operations Director.
Job Functions
The LPN/LVN assesses patient health problems and needs.
Takes vital signs, administers medication, develops and implements nursing care plans, and maintains medical records.
Advises patients on health maintenance and disease prevention or provides case management.
Understand own work area professional concepts, standards, regulations, strategies, and operating standards.
Required Qualifications
Current licence as an LPN or LVN.
Patient facing role in Humana's TB screening program; TB screening required.
Preferred Qualifications
Bilingual in English and Spanish.
Hands‑on professional.
Phlebotomy experience.
Experience with Electronic Medical Records.
Experience with HEDIS.
Knowledge of primary care medicine requirements (state specific).
Working knowledge of clinical operations.
Scheduled Weekly Hours: 40.
Pay Range: $48,900 - $66,200 per year (based on location, experience, education, certifications, etc.). This job is eligible for a bonus incentive plan based on company and/or individual performance.
