Behavioral Health Case Manager
Full‑time position. $18–$21 per hour.
We offer unique and individualized care to children, adolescents, adults, the aging population, those with developmental disabilities, and those suffering from extreme trauma. Clients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers and psychiatrists.
Responsibilities
Provide therapeutic behavioral services to clients by consulting with a licensed clinician to assist with the individual's needs.
Develop and implement individualized support or care plans, coordinating with healthcare, behavioral health, and non‑healthcare services, and creating a treatment plan.
Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
Offer services to individual clients or groups of clients.
Promote a positive and cooperative relationship with all outside contacts.
Requirements
Must be at least 18 years of age.
Bachelor's or Master's level degree in a related field, or a high school diploma with three or more years of relevant experience.
Related experience or training with mental or behavioral health conditions.
Valid driver's license, car insurance and reliable transportation.
Willingness to travel throughout the assigned service area.
