Schedule : Sunday – Thursday, 8:00 A.M. – 5:00 P.M.
Pay : $40.00 per hour + bonus: four payments of $1,000 at 90, 180, 270, and 360 days.

Job Summary
The Wellness Nurse monitors residents’ health and wellness, conducts assessments, initiates service plans, manages care, and collaborates with the care team to keep residents functioning in the assisted living environment.

Position Overview
Meet residents regularly to provide health checks and answer questions.
Review medical reports prior to move‑in to ensure individual needs and regulatory requirements are met.
Order medications and oversee residents’ medication schedules for those receiving medication administration services.
Audit medication records and carts twice weekly; verify shift‑change MAR audit and narcotic count.
Ensure proper documentation of medication assistance administration.
Monitor medication administration records for holes and discrepancies and report to the Wellness Director.
Perform duties following established methods, techniques, and standards.
Assist the Wellness Director with monitoring med passes by the MCM.
Maintain ongoing communication with residents, families, and physicians regarding medical needs.
Coordinate additional health services as required.
On‑call duties for staffing and scheduling; provide coverage during emergencies.
Review changes in residents’ conditions with the Wellness Director, Executive Director, and physicians.
Ensure infection control guidelines are met.
Maintain residents’ health‑care files and document per standards.
Facilitate OSHA training and compliance assistance.
Oversee and assist in resident health and wellness training.
Assist residents with bathing, dressing, grooming, and feeding when necessary.
Assist with resident assessments and negotiated service plans under the Wellness Director’s direction.
Provide safety, comfort, and well‑being to residents.
Direct supervision of caregiving and coordination of health‑care needs within the community.
Perform other duties as assigned.
Qualifications
Demonstrated commitment to core values: Love, Kindness, Honesty, Goodness, Fairness, Respect.
Current LPN license issued by the state and a valid First Aid certificate.
Minimum 2 years of experience working with the elderly in assisted living, skilled nursing, or hospital settings.
Basic typing skills and working knowledge of personal computing and word‑processing software.
Continuing education per law and in‑service training within the community.
Knowledge of procedures and techniques for resident care services and universal precautions.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procurement manuals.
Strong communication skills; able to effectively communicate in English and follow written and oral directions.
Ability to use judgment to recognize symptoms indicative of changes in residents’ conditions.
Eligibility for benefits for team members scheduled 24+ hours per week; families also eligible.
