At Legacy Hospice, we’re committed to caring for patients and families with compassion and dignity. We are seeking a skilled and empathetic Social Worker (SW) to join our team. In this vital role, you'll help patients and families cope with the emotional, social, and practical challenges that come with a terminal diagnosis—bringing comfort and clarity during their most vulnerable moments.

Position
Hospice Social Worker
Location: Leawood, KS
Job Type: Hourly | Non-Exempt

What You'll Do

Conduct psychosocial assessments of patients and caregivers

Provide counseling to patients and families regarding emotional stress, coping, grief, and advance care planning

Assist with advance directives, funeral arrangements, and navigating legal/financial decisions

Identify and coordinate with community resources, agencies, and support organizations

Educate staff and families about cultural and psychosocial needs

Document patient conditions, interventions, and psychosocial support provided

Serve as a liaison between families and care teams to ensure coordinated, holistic care

Participate in interdisciplinary team meetings to develop individualized care plans

Provide crisis intervention and support for safety concerns (neglect, abuse, self-harm)

Participate in quality improvement initiatives and continuing education

Participate in on-call support as needed

Qualifications

Bachelor’s degree in Social Work (BSW) required

Master’s degree (MSW) or LCSW strongly preferred

At least 1 year of experience in social work, preferably in hospice, home health, or a community setting

Knowledge of end-of-life care, grief support, and cultural sensitivity

Strong written and verbal communication skills

Self-motivated, organized, and able to work independently

Current Basic Life Support (CPR) certification

Valid driver’s license, auto insurance, and reliable vehicle required

Legacy Hospice is a drug-free workplace. Pre-employment drug screening is required, including testing for marijuana.
