Summary
The triage nurse’s primary responsibility is to triage telephone calls from clients/patients and caregivers. Using the nursing process, the triage nurse facilitates access to the health care system and/or negotiates a satisfactory resolution to the caller’s problem. The triage nurse is expected to work closely with providers, nurses, and all other members of the organization staff by demonstrating effective communication, teaching, and negotiating skills. The triage nurse is accountable for quality assessment, efficient managed care, and client satisfaction in collaboration with the health care team.
Responsibilities
Maintains the organization’s standards of nursing care in accordance with policies and procedures.
Through the nursing process, using specific protocols and established resources: collects specific data and records information on documentation form, assesses the client’s condition, provides accurate consistent information, formulates and recommends a plan of care, negotiates appropriate nursing care options with the client/patient and caregiver, and validates the care plan with the client/patient and caregiver for completeness and accuracy.
Works collaboratively with other team members as required developing and implementing the plan of care.
Demonstrates excellent communication skills. Maintains a customer-driven professional attitude. Provides non-judgmental advice based on protocols and resources. Demonstrates knowledge and expertise in triaging callers properly such that all callers receive a timely and appropriate response.
Demonstrates skill in client/patient teaching.
Functions independently and demonstrates leadership in decision making, communication, and problem solving techniques to resolve caller’s questions/concerns.
Acts in the role of client and family advocate to assist them through the health care system.
Attends required staff meetings, in services, and/or supervisory conferences as requested.
Performs other related duties as assigned.
Minimum Qualifications
Graduate from an accredited school of nursing
Current New York State Registered Nurse License
Preferred Qualifications
Bachelor of Science in Nursing
Strong analytical, computer, and communication skills essential
Experience in emergency department, ambulatory care or previous telephone consultation experience is helpful
Medical terminology a must
Physical Requirements
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$80,754.00 - $110,292.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
