Job Summary:

Responsibilities
Residents assessments
Care plan development by collaborating with other healthcare professionals
Coordination with IDT to optimize health outcomes
Supervision of CNAs
Compliance with IDPH/CMS

Long Term Care (LTC) experience preferred
