Responsibilities

As a home health nurse (RN HH), deliver healthcare in patients' residences, helping them maintain their independence. Provide one-on-one care for patients in their homes, who may be elderly, critically ill, disabled, or recovering from surgery, injury, or accident.
Assess patients' conditions and determine an individualized care plan.
Chart observations, administer medication and dress wounds, educate patients and their families about care, and coordinate with other medical professionals involved in patients\' care plans.
