We are seeking a Health Information Representative to ignite a new era of impactful health outcomes with data that drives answers, technology that advances performance, and expertise that builds trust. Under general supervision, the Health Information Representative will scan and upload medical records for client’s digital access. Will also be responsible for indexing metadata and ensuring scanned documents are legible and routed to correct department.
Requirements
Essential Duties and Responsibilities
Sort mail to make sure it is in the appropriate department based on assigned Post Office boxes.
Prep mail by removing staples, taping tears, making sure pages are flat, straight and easy to scan.
Index patient or provider social security numbers and other appropriate metadata.
Scan and upload medical records and enrollment packets into DOMA’s digital archiving system for quick viewing and retrieval by the client.
Select correct drop‑down options to ensure patient documentation is routed to the correct department.
Perform quality checks to be sure information is indexed correctly.
Must maintain confidentiality at all times due to possible exposure to PHI & PII.
Qualifications
High school diploma or GED
1-2 years’ experience working with medical records
Proficiency in Microsoft applications such as: Outlook, Word, and Excel
Ability to meet and/or exceed standards in accuracy and productivity
Strong Time Management Skills
Additional Requirements
Attention to detail and data entry accuracy is essential.
Must keep patient information confidential.
Must be able to obtain and maintain security clearance.
Work Environment/Physical Demands

This is an in office position. While performing the duties of this job, the employee regularly works in a climate‑controlled environment.
