We are a locally owned organization with an excellent reputation throughout the San Luis Obispo and Santa Barbara counties. We are constantly growing and now seeking a full‑time Home Health Referral Coordinator (Licensed Vocational Nurse, Medical Assistant or Certified Nurse Assistant) to join our team in our San Luis Obispo office.
The Referral Coordinator is responsible for ensuring that patient care is coordinated and managed efficiently and effectively while working closely with the home health intake team. This position also verifies patient demographics, reviews orders and labs, and communicates with our field clinicians to ensure that patient care and services are delivered appropriately.
The regular work schedule is 8 AM‑5 PM, Monday‑Friday, with rotating 1 weekend day every 4 weeks.
Qualifications
Current California Licensed Vocational Nurse (LVN), Medical Assistant (MA), or Certified Nursing Assistant (CNA)
Home health experience preferred but not required
Ability to maintain a calm, professional demeanor in all situations
Customer‑service–focused approach to all interactions
Strong organizational and communication skills, with the ability to work independently and collaboratively within a team
High attention to detail with excellent follow‑up and ability to complete tasks in a timely manner
Experience with EMR/EHR systems preferred
Ability to coordinate patient care and communicate effectively with clinicians and referral sources
Experience with phone triage and patient communication preferred
