Physical Therapist (PT) Duties
Provide evidence‑based physical therapy services to post‑surgical and orthopedic patients in their homes
Collaborate with surgeons, physician assistants, and care managers to ensure continuity of care
Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes
Educate patients and their families about treatment options, exercises, and preventive measures
Conduct comprehensive evaluations and implement tailored treatment plans
Document all care in alignment with outpatient standards and regulatory requirements
Participate in ongoing case reviews and care team meetings
Work with and supervise PTAs
Physical Therapist (PT) Experience & Qualifications
Current and valid Physical Therapist license in TX
Minimum 1 year of outpatient orthopedic or home health experience preferred but not required
If new grad, training and mentorship will be provided
Strong interpersonal and communication skills
Self‑directed, with the ability to work independently in the field
Reliable transportation and willingness to travel within service area
Excellent communication skills for interacting with patients, families, and interdisciplinary teams
Job Types: Full‑time, Part‑time, PRN, Per diem. Medical Specialty: Orthopedics. Schedule: Choose your own hours. Work Location: In person, visiting clinician.
