All Coastal Health District County Health Department employees are deemed essential employees in the event of an emergency. You may be called to participate in events such as staffing shelters, Specimen Point of Collection Site (SPOC), and/or Point of Dispensing site (POD). Participation may be a part of your job requirement, whether the emergency occurs during or outside regular work hours.
Job Responsibilities
Job duties require home visitation services to medically diagnosed high-risk pregnant, postpartum and inter-conceptual women. The role includes nurse case management to refer for support, education and community resources. Target populations will be outreach-focused to reduce disparities in birth outcomes. General clinical support will be provided in the home. The incumbent will communicate with health care providers with clients’ status updates throughout the program. Must have a valid driver’s license.
Minimum Qualifications
PH Nurse 1
— Associate degree in nursing from an accredited college or university and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification. Salary: $51,811.56
PH Nurse 2
— Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or one (1) year of experience at the lower level PH Registered Nurse 1 (HCP190) or position equivalent. Salary: $58,547.06
PH Nurse 3
— Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification; or two (2) years of experience at the lower-level PH Registered Nurse 3 (HCP192) or position equivalent. Salary: $66,168.17
Preferred: BSN, previous experience in home care and/or medical case management, newborn and/or pediatric. Bilingual-Spanish. One year of Labor & Delivery experience OR one year working in a prenatal care clinic or one year experience working with moms and babies.
