Occupational Therapist
In this role you will provide skilled occupational therapy services on an intermittent basis to clients in their homes in accordance with the physician's orders.

Responsibilities

Assist the physician in evaluating the client's level of function by applying diagnostic and prognostic procedures.

Conduct initial and ongoing comprehensive assessments of the client’s needs, including OASIS assessments.

Evaluate the client’s significant other(s), the home situation, and available family assistance to determine instructional needs and additional agency or community services required.

Ensure physician orders are appropriate and discuss necessary changes.

Assist in development and implementation of the interdisciplinary client care plan to include Occupational Therapy in consultation with the physician.

Guide and instruct the client in prescribed therapeutic self‑care and creative activities that improve independence and physical and mental functioning.

Instruct the client in care and use of wheelchairs, braces, splints, and prosthetic and orthotic devices.

Teach, supervise, and counsel the family and client in the total Occupational Therapy program and related home care issues.

Establish goals based on current functioning and potential for improvement, and perform re‑evaluations as indicated.

Arrange outpatient services when procedures cannot be performed in the home.

Evaluate the effectiveness of intervention, updating the client care plan as needed.

Prepare clinical/progress notes on the day of the visit, incorporating them into the chart within 72 hours.

Communicate with the physician regularly regarding client status and additional orders when necessary.

Communicate with other team members and, when appropriate, instruct them in Occupational Therapy techniques applicable to the client.

Supervise the Home Health Aide every two weeks when Occupational Therapy is the sole professional discipline involved in the client’s care.

Supervise the Occupational Therapy Assistant at least once monthly.

Participate in agency activities, in‑services, and meetings as appropriate.

Confirm weekly scheduling of visits with the Director of Nursing to coordinate agency visits.

Participate with staff, client, and physician in discharge activities and complete an Occupational Therapy Discharge Summary.

Commit to self‑development through educational programs, workshops, conferences, professional organization involvement, and research.

Understand and adhere to agency policies and procedures.

Participate in Quality Assessment and Performance Improvement activities as assigned.

Attend all patient care conferences as scheduled.

Complete and submit OASIS assessments, reassessments, transfers, resumptions, discharges, visit notes, and significant changes in condition in accordance with agency-defined time frames.

Obtain medical history relevant to the present condition.

Qualifications

Graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association.

Member of the American Occupational Therapy Association is preferred.

Licensed or registered by the state.

At least one year of institutional experience.

At least two years of home care experience and at least one year of supervisory experience (two years preferred).
