Mobile Medicine Model:Provide care out of fully equipped medical vans serving locations throughout the Sacramento region including homeless shelters, community centers, and senior centersVans include patient rooms and are staffed with medical assistantsWork alongside physicians and other APPsIntegrated services include primary care, behavioral health, and dental care

Patient Volume & Workflow Expectations:3 patients per hour model (20-minute appointment slots)Includes telehealth visits when appropriateAdministrative time built into the end of the dayProviders typically average approximately 2.2 patients per hour accounting for no-showsNo double-booking

EMR: Epic (experience highly preferred)

Candidate Requirements:Board Certified or truly Board Eligible NP/PAMinimum two years post-training experience requiredFQHC or community health outpatient clinic experience preferredComfortable practicing in a 3 patients per hour modelAbility to complete charts within 72 hours requiredConfidence and proficiency managing in-basket responsibilities including patient messages, medication refills, and lab resultsExperience utilizing translation services is helpful (more than 48% of patients speak a language other than English)Must be COVID-19 vaccinated
