
Home Health Hospice Nurse

Evergreen At Home - Home Health is looking for a Registered Nurse to join our team! The RN works in collaboration with a team of clinicians to provide skilled nursing services to patients in their own homes and communicates regularly with the patient's physician to provide critical updates about changes in the patient's medical condition.
The available position is 30-40 hours per week, mostly during the day, and includes on-call responsibilities (every third week). It is also eligible for benefits, including health, dental, and vision insurances, vacation, 403(b) retirement plan matching, and much more!
Responsibilities
Coordination of services and care for assigned clients (including therapy)
Observation, recording, and reporting signs and symptoms, reactions to treatments and medications, and any change in the client's physical or emotional condition in a timely manner
Teaching the client and family members about proper care and effective management of medical conditions
Arrangement of counseling services for the client and family as appropriate
Accurate completion of OASIS documentation
Discharge planning and preparation of discharge summaries and instructions
Minimum Requirements
Valid State of Wisconsin Registered Nurse license
Current CPR certification
Valid State of Wisconsin driver license
Reliable vehicle and appropriate insurance
Acceptable motor vehicle record
Previous experience managing chronic conditions a plus
Passion for working with older adults
Strong prioritization and organization skills
