Job Duties & Skills Required
What You Will Do
The IDT Care Coordination Specialist is responsible for preparing for and facilitating all Interdisciplinary Team (IDT) meetings to ensure regulatory compliance.
This role ensures that all IDT regulatory requirements are monitored and completed within the regulated timeframes, IDT agendas are structured and distributed in advance, and care planning sessions are scheduled and executed efficiently. This role supports and lives out Immanuel’s Mission and CHRIST Promises.
Skills & Requirements
Education
Bachelor’s degree in healthcare administration, nursing, social work or related field is required. Equivalent years of experience may substitute for educational requirement.
Experience
Three (3) years’ experience in health care/long-term care operations, care coordination, or program management. Experience in PACE or managed care is preferred. Experience in workflow tracking systems preferred. Experience within an interdisciplinary team model preferred.
One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
Knowledge, Skills and Abilities
Strong project management and organizational skills.
Proficiency in EPIC electronic health record system.
Proficiency in Microsoft Teams, Excel, Word.
Proficiency in Smartsheet.
Understanding of CMS regulatory timelines and audit expectations.
Proactive communication.
Systems thinking abilities.
Skills in detail orientation.
Process improvement mindset.
Ability to manage competing priorities.
