We are looking for
We are seeking a Licensed Clinical Social Worker (LCSW) to provide specialized mental health counseling for social and emotional adjustment during rehabilitation from catastrophic injury or illness.
This position involves supporting individuals and families as they adapt to disability and lifestyle changes.
You will provide therapeutic interventions, as well as resource support and navigation that address the patient’s goals.
Knowledge of medical social work and/or neurological injury (TBI/CVA).

Responsibilities
Conduct comprehensive evaluations, assess the social, emotional and support needs of patients and families, set measurable objectives and goals formulated with the rehabilitation team, adjust treatment plan for maximum results.
Identify and assist patients and/or families to use appropriate community resources to achieve measurable objectives, resolve adverse social and emotional responses connected with the injury/illness.
Provide direct social work intervention, instruct and counsel patients and family members in coping with social and emotional responses related to the injury/illness.
Regularly re‑assess effectiveness of treatment plan, progress towards goals and need for modifying goals or treatment.
Communicate patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicable.
Consult with medical providers regarding patient evaluation & treatment, progress and discharge from treatment.
Observe, document and report changes in status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other team members.
Maintain and submit evaluation, treatment and billing documentation per company, facility, funding source, local, state and federal regulations.
Adhere to standards of the company’s code of business conduct, policies, HIPAA and corporate compliance programs, and professional practice standards and applicable laws.
Perform other duties as assigned.
Qualifications
Master’s Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC).
Current licensure/certification in the state(s) of practice required.
Minimum one year of experience in a rehabilitation setting serving individuals with TBI, spinal cord injury or other acute diagnoses.
Current BLS First Aid and CPR certifications.
Effective professional interpersonal, verbal, and written communication skills.
Ability to work independently and as part of a team.
Strong organization skills and attention to detail.
