PATIENT POPULATION & SCOPE
• Patient Demographics: Adults and Pediatrics
• Average Daily Volume / Census: 38-40 patients per day
• Consults & Follow-Ups: Yes
• Case Mix/Clinical Focus Areas: Comprehensive Ophthalmology including Cataract, Glaucoma, Oculoplastics, Medical Retina, Pediatric, and Cornea
FACILITY & TEAM DETAILS
• EMR System: Cerner
• Specialty Backup: Established referral base from primary care network
• Support Staff: 2 Ophthalmologists, 2 Optometrists (including low vision specialist), dedicated technicians and scribes
• Leadership & Reporting Structure: Physician-led multispecialty group
• Reason for Opening: Growth

• Board Requirements: BC/BE in Ophthalmology
• DEA & Certifications: DEA (as required)

• State License Required: Active WI License or Eligibility to Apply
• Certifications: As required
• Experience: Open to new graduates and experienced physicians
• Additional Requirements: Interest in comprehensive ophthalmology practice
DUTIES & RESPONSIBILITIES
• Provide comprehensive ophthalmologic care across a broad range of conditions
• Perform surgical and non-surgical eye procedures
• Collaborate with optometrists and multidisciplinary care teams
• Maintain continuity of care through follow-ups and consults
• Maintain accurate and timely medical documentation
