RN Case Manager – Inspire Hospice and Palliative Care
We’re looking for a compassionate, dedicated RN Case Manager to provide care to patients in homes, assisted living, or skilled nursing facilities. The individual must have a valid GA RN license, a driver’s license, and at least one year of experience in hospice or home health care.
Responsibilities
Provide comfort and support to patients and families.
Educate families about disease progression, self‑care, and nutrition.
Collaborate with the interdisciplinary hospice team to develop and implement patient care plans.
Manage symptoms and advocate for patient comfort.
Maintain timely and accurate clinical documentation.
