Hospice Admission Coordinator

Gentiva - - Responsibilities: Serve as the primary contact for new referrals and coordinate all aspects of patient admission within the companys 3-hour standard.; Communicate promptly with patients, families, referral sources, and internal clinical staff to ensure a seamless admission experience.; Obtain and verify insurance information, complete payer source verification, and assist billing staff to ensure accurate and timely reimbursement.; Participate in daily team meetings, marketing initiatives, and community education events to strengthen relationships with referral sources.; Maintain accurate admission and referral data in the companys information system.
