Case Manager-HCH

Responsibilities

The Case Manager-HCH is an essential member of the Healthcare for the Homeless (HCH) and Street Medicine Team. This position involves outreach and engagement with homeless individuals in various environments, such as encampments, abandoned buildings, parks, meal programs, bus stations, streets, alleys, and under bridges. The Case Manager promotes awareness of and access to opioid treatment services and provides logistical support for health care enrollment, housing, and other social services. This role combines case management and peer support, focusing on connecting homeless individuals to essential services while advocating for their rights and well-being.
Develops and maintains familiarity with the homeless population, including their locations, movements, social and physical environments, and resources.
Learns and maintains knowledge of the locations and movements of homeless individuals and the social and physical environments, problems and resources used, as well as other patterns or characteristics of the sub-culture.
Educates homeless individuals about available services and refers them to relevant programs, including opioid treatment services and HCH.
Provides logistical support with enrollment for health insurance programs, housing, and other social services including filling out paperwork, reminders for appointments, transportation to appointments, medication pickup, etc.
Maintains contact with homeless individuals who may be physically or mentally ill but initially refuse services, offering ongoing support and assistance.
Develops and maintains knowledge of relevant public and private agencies and service providers in Bakersfield to facilitate referrals and linkages to services.
Maintains adequate records as to outreach activities, advocacy efforts in the community, and fulfills all other documentation and tracking requirements as required by administration of grants for supportive services.
Serves as an advocate and educator, promoting the dignity of individuals and ensuring they have access to the information and resources they need for their healthcare and well-being.
Provides other necessary services to homeless individuals, as appropriate and required by the team.
Upholds the mission and policies of Clinica Sierra Vista, ensuring high-quality, compassionate care for all individuals.
Other duties as assigned.
Qualifications

Education: High school diploma or GED equivalent
Experience working with homeless population and persons from diverse cultures and income levels.
Bilingual proficiency in second language is preferred.
Intermediate skill level in the use of a PC and related software applications (MS Office, Access and Excel)
Excellent verbal, written communication and listening skills.
Strong interpersonal skills with a demonstrated ability to work independently on individual and assigned tasks while exhibiting good judgment.
Demonstrated ability to maintain confidentiality.
Ability to manage multiple competing priorities and work well in a fast-paced environment.
Understanding of basic healthcare principles with the ability to make reasonable and appropriate referrals.
Ability to work as a team member.
Reasonable and predictable in person attendance.
Ability to work some evenings and weekends.
Must pass a criminal background clearance.
Travel within Kern county- must have a valid California Driver\'s License and a properly insured vehicle for use on the job.
Current TB clearance.
Must be able to carry forms and supplies up to 50 pounds.
