Position Overview
The Home Health Registered Nurse provides skilled, patient-centered nursing care to individuals in their homes in accordance with physician orders and individualized plans of care. This role requires strong clinical judgment, independent decision-making, and effective care coordination to support safe, high-quality outcomes across the assigned service area.
Schedule & Work Details
Schedule: Monday through Friday
Hours: 8:00 am to 4:30 pm EST
Coverage Area: Oakland, Wayne, Livingston, Washtenaw, Macomb, and Genesee counties
On Call Requirement: One day per week and one weekend per month
Mileage Reimbursement: Yes, reimbursed between visits from first to last patient
Pay Rate: $51 to $53 per hour
Key Responsibilities
Comprehensive In-Home Patient Assessment
Conduct detailed initial and follow-up assessments, including physical condition, vital signs, and symptom evaluation
Assess medication usage, adherence, and effectiveness
Evaluate home safety, environmental risks, and patient ability to perform activities of daily living
Development and Management of Individualized Care Plans
Develop, implement, and update individualized plans of care
Ensure alignment with physician orders, agency policies, and regulatory requirements
Modify care plans in response to changes in patient condition
Direct Skilled Nursing Care
Administer medications, including injections and IV therapy when ordered
Provide wound care and monitor healing progress
Perform post-hospital and post-surgical follow-up care
Manage chronic conditions such as diabetes and heart failure
Ongoing Monitoring and Clinical Evaluation
Monitor patient progress and response to treatment
Identify early signs of clinical deterioration or complications
Communicate changes promptly to physicians and interdisciplinary care teams
Patient and Caregiver Education
Educate patients and caregivers on medication management and side effect monitoring
Provide instruction on disease processes, symptom recognition, and self-care techniques
Promote infection prevention, safety, and independence in the home
Care Coordination and Interdisciplinary Collaboration
Collaborate with physicians, therapists, social workers, and home health aides
Ensure continuity of care across disciplines
Facilitate referrals and additional services as patient needs evolve
Clinical Documentation and Regulatory Compliance
Complete accurate and timely clinical documentation
Maintain compliance with Medicare, CMS, and agency standards
Support audits and quality reviews through thorough record keeping
Patient Advocacy and Independent Practice
Advocate for patient needs, preferences, and quality of life
Provide emotional support to patients and families
Prioritize workload independently and make sound clinical decisions in non-acute settings
Required Qualifications
Active, unencumbered Registered Nurse license in the state of practice
Minimum of one year of home health nursing experience
Required experience using HomeCare HomeBase (PointCare) EMR
Ability to work independently and perform autonomous assessments
Valid driver’s license and reliable transportation
Current Basic Life Support (BLS) certification
