Wellness Nurse (LPN)

Job Description
Led by the community's Health and Wellness Director (RN), the Wellness Nurse is a Licensed Practical Nurse (LPN) who provides direct nursing care to residents with an emphasis on holistic wellness. The role includes supervising the daily nursing activities of home health aides (HHAs) and participating in the creation of the community’s foundation and culture as we open for residents.

Schedule: Part‑time (22‑29 hours per week) non‑exempt, weekdays and alternating weekends. Day shift: 7:00 am–3:00 pm; Overnight shift: 11:00 pm–7:00 am; Weekends: 3:00 pm–11:00 pm.

Salary range: $31–$35 per hour.

Responsibilities and Duties

Monitor the health, safety, and well‑being of all residents.

Assist in training and monitoring medication administration.

Supervise care staff in accordance with state regulations and community policies.

Maintain clinical quality assurance in accordance with federal, state, and local standards.

Promote the highest degree of service to residents while demonstrating the company’s mission.

Conduct thorough resident assessments.

Qualifications

Graduated from an accredited School of Nursing with a current LPN license.

At least 2 years of experience in long‑term care, assisted living, home health, or hospital settings (preferred).

Experience working with residents with Alzheimer’s or other related dementias.

Ability to handle multiple tasks.

Knowledge of federal and state regulations and nursing practices applied to health and wellness residents.

Proficiency in computer skills and Microsoft Office (Windows, Outlook, Excel) and ability to learn new applications.

Physical ability to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.

Required to lift or carry up to 50 lbs and perform intermittent physical activities including lifting and supporting residents. Use proper body mechanics.
