Position Summary
Clinical Case Managers provide comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each person we serve's abilities to function in the community. They assess, monitor, and follow up with our clients to meet basic needs and move toward recovery. They are responsible for all clinical documentation of consumer interaction and provide transportation in their vehicle (additional cost for insurance reimbursed).

Compensation & Benefits

CCMI $30.12/hr

CCMII $31.25/hr

Language differential +$1.00-$2.50/hr

Job Duties & Functions

Help people we serve in meeting basic needs for food, clothing, shelter, personal safety, and general medical and dental care, and assist them with applications for income, medical, housing, or other benefits which they may need and to which they are entitled. Identify those who need a protective payee‑ship and arrange this assistance.

Ensure access to needed services and community resources by arranging transportation, or by taking them to the service in personal vehicle.

Provide comprehensive psychosocial services that include a continuum of high and low intensity services designed to improve or maintain abilities to function in normal social roles. These services include, but are not limited to:

Help evaluate strengths and symptoms and facilitate goal setting and service planning.

Guide and instruct in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, and use of transportation.

Help develop social skills, interests, and leisure activities to provide participation and personal worth, including culturally appropriate activities.

Help find and use appropriate employment opportunities, vocational rehabilitation services or supported work environments.

Assist with special living arrangements in locating rehabilitative or supportive housing.

Offer support, assistance, consultation, and education to families, friends, landlords, employers, community agencies, and others to maximize benefits and minimize problems.

Identify and work with potential natural support systems such as neighborhood networks, churches, and community organizations.

Make full utilization of consumer's support systems to encourage treatment program engagement.

Reach out to program‑eligible people, inform them and educate them about available services at WellPower and in the community.

Maintain a trauma‑informed environment of wellbeing.

Perform other duties as assigned.

Requirements & Qualifications

Education: Bachelor (CM I) or Master (CM II) Degree in psychology, social work, or related human services degree.

Experience: One year in mental health or human services preferred.

Must have valid Colorado Driver's license and a clean driving record. Ability to transport clients in your vehicle and obtain and carry Business Use coverage on your auto policy.

Shift / Hours
This position is Monday‑Friday, observing standard business hours.

Physical Requirements
Ability to sit, stand, bend and reach; drive a vehicle; operate standard office equipment such as phone, computer, fax, copier/printer, and other business machines. Occasional lifting up to 40 lbs.
