
SSYI Outreach Worker/Case-Manager (Full-Time)

Overview
The SSYI (Safe and Successful Youth Initiative) Outreach Case Manager works with high-risk young adults to connect them with essential services, with the ultimate goal of reducing violence and criminal activity. The role involves building trusting relationships, conducting assessments, and coordinating a personalized intervention plan.
Essential Functions
Outreach & Engagement
Identify, build trust with, and engage proven-risk youth through street outreach and community presence.
Conduct consistent outreach during non-traditional hours (evenings/weekends).
Serve as a mentor, advocate, and credible messenger to SSYI participants.
Conduct risk
eeds assessments and assist with intake and enrollment into the SSYI program.
Case Management & Support Services
Develop and maintain individualized service/success plans in collaboration with participants.
Provide direct support and resource navigation for housing, education, job training, employment, mental health, substance use, and legal support.
Maintain regular contact with participants (in-person, phone, etc.), tracking progress toward goals.
Facilitate or coordinate access to counseling, therapeutic, and trauma-informed services as needed.
Collaboration & Coordination
Coordinate closely with local law enforcement, schools, employers, healthcare providers, and community-based organizations in support of participant needs.
Participate in regular multi-agency case conferencing meetings to assess participant risk and progress.
Collaborate with outreach and reentry teams, youth service providers, and program evaluators.
Advocate for youth across systems (criminal justice, healthcare, education, etc.).
Documentation & Compliance
Maintain accurate, timely documentation in program databases (e.g., referral forms, case notes, assessments).
Ensure compliance with grant requirements and confidentiality laws (HIPAA, FERPA, etc.).
Contribute to program evaluations by providing data and success stories as requested.
Qualifications
High school diploma or equivalent (GED); associate or bachelor s degree in social work, human services, criminal justice, or related field preferred.
Minimum 1–2 years of experience in youth outreach, street work, reentry services, or case management.
Strong connection to and knowledge of the local community.
Demonstrated ability to engage high-risk youth and young adults.
Cultural competency and trauma-informed care experience.
Valid drivers license, reliable transportation, and willingness to travel locally.
Ability to work flexible hours, including some nights and weekends.
Commitment to public safety, racial equity, and youth development.
Commitment to the YMCA mission, values, and leadership competencies, with a willingness to obtain Multi-Team/Branch Leader certification within the required timeframe.

Job Type: Full-Time (Non-Exempt)
Schedule: Monday-Friday 9:00am-5:30pm
Pay: $28.00/hr with a full benefit package to include generous PTO, access to facilities, Medical, Dental, Disability, Life, Vision, & Pet Insurance, and ability to participate in a 403B Retirement Savings Plan. Also, once fully vested, the YMCA will pay into a retirement account for you.
