Adult Day Care Social Worker & Care Planner

Centers Health Care is seeking a dedicated Social Worker/Case Manager for their Adult Day Care in Brooklyn, New York. The ideal candidate will be responsible for maintaining and improving residents' physical and mental well-being, identifying social and emotional needs, and developing personalized care plans.
