Job Responsibilities

Provide comprehensive assessment and diagnosis of behavioral health patients
Assist providers in recognizing and treating mental, psychosocial, and substance abuse disorders
Develop effective treatment plans and support patients in achieving goals
Evaluate crises and apply appropriate interventions and referrals
Provide assessment, consultation, and brief intervention for psychological/psychiatric disorders
Maintain an active presence and communicate with primary care providers during clinic hours
Be available for same‑day and scheduled initial interventions with patients
Conduct brief, limited follow‑up visits for selected patients
Participate in meetings supporting IHC’s integrated healthcare model for comprehensive patient care
Detect at‑risk patients and develop plans to prevent deterioration
Support primary care teams in developing care management processes (guidelines, disease management, case management, patient education) to improve chronic disease and substance use self‑management
Educate patients, families, and staff on prevention and treatment enhancement techniques
Monitor the site’s behavioral health program, identify service problems, and recommend improvements
Desired Skillset

Excellent knowledge of behavioral medicine and evidence‑based treatments for medical and mental health conditions
Comfortable operating in a primary care setting within an interdisciplinary team
Ability to design and implement clinical pathways and protocols for chronic conditions
Proficiency in brief patient contacts and accurate clinical assessments
Strong communication skills
Cultural awareness and sensitivity
Good knowledge of psychopharmacology
Requirements

Master’s degree in social work from an accredited university or college
LCSW, LMFT, or LCMHC licensure in the state of Indiana
Basic Life Support (BLS) certification (including AED)
Active and clean Indiana driver’s license with state‑mandated auto insurance
At least one year of experience; three years or more preferred
