Responsibilities
Position involves professional nursing work as a member of an integrated team, providing all aspects of care to HIV/AIDS clients. The nurse ensures that all clinical services provided are consistent with recommended protocols, of the highest quality, and that clients are enrolled appropriately. The role functions with a high degree of independence, working under established protocols and utilizing nursing assessment skills.

The nurse assists with orientation and initial patient education and needs assessment at enrollment, creates nursing care plans, and works cooperatively with the Part B case manager to ensure each client accesses comprehensive services according to the individualized plan of care. Responsibilities also include maintaining accurate client medical records, tracking laboratory results, and providing follow‑up services to clients exiting the program or lost to care. Phone assessments and triage are conducted as needed.

Additional clinical duties include preparing patients and assisting with examinations, ordering, interpreting, and evaluating diagnostic tests, and coordinating with physicians regarding patient conditions. The nurse discusses cases with physicians, informs them of patient conditions, and instructs clients on health education, medication adherence, disease transmission prevention, and develops health improvement plans. The nurse monitors quality of care and compliance with state and federal mandates and assists with the development of policies and procedures for the Ryan White Program.

As nurse case manager, the nurse determines appropriate specialty referral resources in collaboration with the physician, arranges consultations, contacts and arranges referrals with contracted service providers, monitors and tracks all referrals, and documents all activities in the medical record or computer data management system. The nurse also assists the FDOH‑Okaloosa Linkage Coordinator with new HIV cases and linkage to care efforts, enrolls new patients, and maintains an accurate client database as per state policy and procedures.

The nurse coordinates all outreach components of the HIV/AIDS program, collaborates with test sites, providers, and community groups, assists with special events, and coordinates care for new HIV positive clients with surveillance officers, DIS, and out‑of‑state agencies. The nurse participates in quality improvement activities, provides community education, attends staff meetings, case reviews, and in‑services, and performs all other duties as assigned in support of the Agency’s mission.

Required Knowledge, Skills, and Abilities

Knowledge of nursing principles and techniques, human anatomy and physiology.

Ability to work with patients, interpret and follow medical instructions, observe symptoms and reactions, demonstrate manual dexterity, and administer medications.

Ability to communicate effectively and establish and maintain effective working relationships with others.

Knowledge of computer functions and use of programs in a health‑care setting.

Possesses problem‑solving and analytical skills.

Communicates effectively in writing and speaking.

Knowledge of the principles and processes of strategic planning, resource allocation, leadership techniques and behavioral competencies.

The ability to manage and control emotional reactions and conflict in a productive manner.

Qualifications

Licensure as a Registered Professional Nurse according to Chapter 464, Florida Statutes.
