Job Description Summary
This is a full‑time, day shift position in Ambulatory and Anticoagulation care at Mansfield Hospital. The schedule is 4 10‑hr shifts per week (Monday, Tuesday, Thursday, and Friday – these days are non‑negotiable). Residency and/or previous experience is preferred.

Job Summary
This position is responsible for evidence‑based therapy management and care for patients in the ambulatory setting under collaborative practice agreements (primarily for anticoagulation but likely including other disease states). The pharmacist provides patient‑centered education throughout therapy, delivers appropriate instruction and dosing guidance based on laboratory data, reviews individualized treatment plans, and ensures patient adherence to medication therapy. The goal is to minimize risks associated with medication therapy through education, monitoring and communication with patients and providers. Pharmacists collaborate with physicians, nurses, and other health‑care practitioners to provide safe, high‑quality, effective drug therapy. Pharmacists will serve as preceptors for pharmacy students and residents in their area of training and expertise.

Responsibilities And Duties
Acute Care/Infusion
50%:

Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug‑drug interactions or allergies. Contacts providers to clarify orders as appropriate. Dispenses and compounds medications and drug therapy‑related devices in accordance with standards of practice and OhioHealth pharmacy policies, ensuring all medications, chemicals, and supplies are dispensed with integrity. Ensures appropriate labeling and interprets prescriptions for compatibility. Prepares care plans to ensure optimal therapeutic outcomes and maximize compliance.

20%:

Manages individuals’ drug therapy by initiating, monitoring, and modifying prescriptions in a consulting relationship with a provider. Provides drug information to providers, other health‑care professionals, and patients as needed.

10%:

Counsels individuals regarding drug therapy, recommends drug therapy‑related devices, assists in the selection of non‑prescription drugs for common diseases and injuries, and instructs proper use.

10%:

Provides associate and learner development and training, including precepting students and residents, and delivers educational programs for pharmacy, nursing, and medical staff.

10%:

Directs and guides pharmacy technicians and support personnel.

Ambulatory Care/Population Health
50%:

Manages individuals’ drug therapy by initiating, monitoring, and modifying prescriptions in collaboration with a provider. Prepares individualized care plans, provides drug information to providers and other health‑care professionals, and delivers patient education.

20%:

Partners with patients, caregivers, and other health‑care professionals to prevent, manage, and/or slow disease progression through advocacy, wellness promotion, triage, referral, education, and self‑management.

10%:

Assists with guideline/policy development and implementation and participates in performance improvement initiatives.

10%:

Provides associate and learner development and training, including precepting students and residents, and delivers educational programs for pharmacy, nursing, and medical staff.

10%:

Directs and guides pharmacy technicians and support personnel.

Retail
50%:

Reviews provider orders for drug choice, dosage, schedule, and interactions or allergies. Contacts providers to clarify orders. Dispenses and compounds medications and device therapies. Ensures proper labeling and billing procedures.

20%:

Counsels individuals about drug therapy, recommends devices, assists in selecting non‑prescription drugs for common diseases and injuries, and instructs on proper use.

10%:

Manages individuals’ drug therapy by initiating and modifying prescriptions in collaboration with a provider.

10%:

Provides associate and learner development and training, including precepting students and residents, and delivers educational programs for pharmacy, nursing, and medical staff.

10%:

Directs and guides pharmacy technicians and support personnel.

Medication Safety, Stewardship, & Utilization
55%:

Leads safety, stewardship, and utilization strategies for designated site(s) in collaboration with the system team. Facilitates activities of site and system committees and clinical guidance councils. Develops, enforces, and maintains policies that promote and enhance patient outcomes through safe and cost‑efficient medication use. Identifies opportunities for proactive change to improve outcomes and integrates improvement initiatives with clinical and operational programs. Ensures compliance with evidence‑based recommendations and regulatory standards.

20%:

Provides associate and learner development and training, including precepting students and residents, and delivers educational programs for pharmacy, nursing, and medical staff.

10%:

Directs and guides pharmacy technicians and support personnel.

5%:

Reviews provider orders for drug choice, dosage, schedule, and interactions or allergies in various pharmacy practice settings, ensuring all medications and supplies have integrity, proper labeling, and compatibility. Prepares care plans to ensure optimal outcomes and compliance.

5%:

Manages individuals’ drug therapy in multiple care settings and provides drug information as needed.

5%:

Counsels individuals on drug therapy, recommends devices, and instructs on proper use across care settings.

Minimum Qualifications

Bachelor's Degree (Required)

Ohio Pharmacists License (OPL) – Ohio Board of Pharmacy

Additional Qualifications
BS or Doctor of Pharmacy degree from an accredited institution and a current Ohio pharmacist license. Completion of a Pharmacy Practice residency or equivalent ambulatory experience.

Specialized Knowledge
Independent drug dosing under consult agreement.
