Physical Therapist Position
Elara Caring is hiring Physical Therapists to provide high‑quality physical therapy services in a home care setting. The role involves delivering direct patient care, designing treatment plans, documenting progress, leading support staff, and serving as a community resource.

What You’ll Do as a Physical Therapist
Promote Elara Caring’s mission and provide top‑tier therapy services
Deliver direct patient care and evaluate functional needs & outcomes
Design customized treatment plans based on patient assessments
Adjust care plans to ensure optimal recovery and independence
Lead and guide support staff
Document goals, progress, and updates clearly and professionally
Serve as a valuable resource to the community and team
Requirements
Graduate of an APTA‑approved Physical Therapy program
Current, unrestricted PT license in the state of work
Minimum 1 year of experience in a healthcare facility
Reliable transportation & valid driver’s license
Willing to travel up to 50% within coverage area
Ability to lift/move 50–100 lbs and perform physical duties
You’ll report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
