Pharmacist Resident

Job Summary and Responsibilities
As our Resident Pharmacist, you will complete an intensive post‑graduate training program, developing advanced clinical skills and expertise to deliver patient‑centered pharmaceutical care and optimize medication safety. Every day you will rotate through diverse clinical settings, providing direct patient care under supervision. You'll evaluate medication regimens, make evidence‑based recommendations, monitor therapy, and educate. Responsibilities include interdisciplinary participation, research, and contributing to quality initiatives. To succeed, you'll need exceptional clinical knowledge, strong critical thinking, excellent communication for team collaboration/education, and a profound commitment to learning, patient safety, and advancing pharmacy practice. Self‑motivation and adaptability are essential.

Participate in all aspects of comprehensive clinical pharmacy services including rounds with medical teams, pharmacokinetic consultations, parenteral nutrition consultations, anticoagulation management, antimicrobial stewardship monitoring, medication reconciliation, discharge counseling, documentation of clinical interventions in the identified database, and compliance with clinical pathways and protocols.

Perform all activities associated with meeting the goals and objectives of the American Society of Health‑System Pharmacists (ASHP) accredited PGY1 residency program.

Process prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe and accurate manner to ensure the appropriateness of the indication, dose/frequency, route, interactions, dose adjustment, physical and therapeutic incompatibilities, side effects, duplicate therapies, and cost‑effectiveness. Evaluate the need for all non‑formulary medications required.

Provide direct oversight and timely and accurate verification of final product for all aseptic parenteral and non‑parenteral medication preparation, dispensing and distribution in compliance with state, federal statutes and hospital procedures.

Ensure doses are filled and delivered within established departmental guidelines in a timely and safe manner.

Identify and evaluate adverse drug reactions and medication errors.

Pay Range
$1.00 - $750.25 /hour
