Summary
The Community Living Center (CLC) Admission Coordinator Registered Nurse provides care and counsel to persons, conducts systematic data gathering and assessment, applies appropriate nursing judgment, and evaluates human responses through services such as case finding, health teaching, and health counseling. The role also includes the provision of supportive or restorative care to enhance life and well‑being, and the execution of medical regimens including administering medications and treatments.

Qualifications
Basic Requirements

English language proficiency – in accordance with 38 U.S.C. 7403(f) no person shall serve in direct patient care positions unless proficient in basic written and spoken English.

Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by either the ACEN or CCNE at the time the program was completed.

Or individuals attending a master’s level bridge program who have completed coursework equivalent to a bachelor’s level degree in nursing may be eligible for registration before completing the bridge program.

Upon achievement of a state license, the individual may be appointed on a temporary basis and later converted to a permanent appointment after completing and graduating from the bridge program.

Or, for graduates of foreign nursing schools, possession of a current, full, active, and unrestricted registration will satisfy the requirement for an approved professional nursing school graduate.

Credit for foreign nursing education higher than associate/entry level requires formal degree equivalency validation from a recognized equivalency evaluation accepted by the VA, such as International Consultants of Delaware (ICD).

Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state, territory, or the District of Columbia.

Graduate Nurse Technician (GNT) exception: Candidates meeting the basic education requirements but lacking licensure may be appointed at the entry step of the grade and level applicable, with a 120‑day temporary appointment while pursuing licensure (extendable up to two years on a case‑by‑case basis). Grandfathering provision applies to current VHA 0610 series employees.

Preferred Qualifications

Minimum of 5 years of clinical experience in home health, long‑term care, rehabilitation, or admissions/intake coordination.

Physical Requirements

Agrument: Visual acuity, keen hearing, clear distinctive speech, and manual dexterity.

Potentially long periods of walking, standing, stooping, sitting, bending, pulling, and pushing; transferring patients and objects may be required; mechanical lifts are provided.

Risk of exposure to infected patients and contaminated materials; may require donning protective clothing in isolation or during operative/invasive procedures, or in home settings.

Occasional exposure to combative patients secondary to delirium, dementia, or psychiatric disorders.

Must complete annual Employee Health requirements as a condition of employment.

May be required to drive or ride in GSA vehicles when providing care in home settings or off‑site locations.

Duties

Assist with patient planning assessments, identify barriers, and determine activities required for successful transition into the CLC setting.

Apply the nursing process – systematic data gathering, assessment, and appropriate nursing judgment – to ensure positive patient outcomes and monitor bed availability.

Serve as the point of contact for the outcome of screening decisions.

Deliver fundamental knowledge‑based care to assigned clients while developing technical skills.

Assess, plan, implement, and evaluate care based on age‑specific components.

Coordinate care with a focus on patient education, self‑management, and customer satisfaction throughout the continuum of care.

Administer medications and procedures per established policies and guidelines.

Influence care outcomes by collaborating with interdisciplinary team members.

Collaborate with the interdisciplinary team to identify veterans appropriate for admission to the CLC.

Act as an active member of the interdisciplinary team whose primary focus is CLC screening and placement.

Collaborate with healthcare system members and stakeholders to provide a seamless, timely referral system for veterans seeking CLC admission.

Ensure veterans are placed in program areas that best meet their needs.

Collect and maintain data that will enhance program planning and placement.

Initiate consultations for complex administrative and clinical issues.

Collaborate with VA and non‑VA departments, community agencies, and healthcare providers to ensure patient needs are addressed.

Perform all other duties assigned that pertain to this occupation to meet the needs of the agency, department, facility, and/or service.
