LVN – Adult Hospice and Home Health
Job Title: LVN – Adult Hospice and Home Health
Locations: Lower Valley & Upper Valley
Job Types: Part-Time, PRN
Shifts: Day Shift, Evening Shift, Night Shift, and Weekends as needed
Work Schedule: Monday to Friday, Weekends, Evenings as needed
Key Responsibilities
Provide direct nursing care to patients in their homes, ensuring implementation of care plans in line with physicians' orders.
Regularly monitor vital signs, track medical conditions, document changes, and maintain accurate health records.
Monitor patients’ medication usage and report results, ensuring proper administration and dosage.
Work closely with the interdisciplinary team, patients, and their families to assess, implement, and evaluate care plans.
Assist with emotional, personal, and medical support for patients, providing holistic care to meet their needs.
Evaluate patient behavior, diagnose conditions, and consult with healthcare professionals to improve patient outcomes.
Working Conditions
Work in patient homes, often in varying environmental conditions (including possible exposure to bodily fluids and infectious diseases).
Ability to travel locally to patient homes and manage flexible work hours.
Occasional emergency PRN calls and exposure to adverse weather.
Reasonable accommodations may be made for individuals with disabilities.
Qualifications
Education: Graduate of an accredited school of vocational nursing.
Current State LVN License.
BLS Certification (required).
Valid Driver’s License.
Minimum of 2 years as an LVN in Clinical, Hospice, or Home Health settings (preferred).
