Job Overview
Location: 2822 East Colfax Ave. This is a full-time, on-site position.
Salary: Pay: $24.00 - $34.00 per hour • Pay Frequency: Bi-weekly
Education/Licensure: One of CAT/CAS, LAC/LCSW/LPC, or a Masters in Social Work/Behavioral Health (Required). License/Certification: One of CAT/CAS/Master/License (Required).
Schedule
There are two schedule options:
Wednesday - Saturday 5:00 am to 1:30 pm; Sunday 7:00 am to 3:30 pm
Saturday 5:00 am to 1:30 pm; Sunday 7:00 am to 3:30 pm, Monday - Wednesday 5:00 am to 1:30 pm
Responsibilities
Provide direct/indirect therapy services to outpatients who require or benefit from therapy skills and interventions
Provide case management to coordinate care with primary care doctors, other prescribing physicians, probation/parole, child/adult services, and other supporting services
Attend all clinic team meetings and company trainings
Maintain client records and other data as required by regulatory agencies promptly
Treatment Planning: apply assessment, evaluation, and reassessment data to form treatment plans
Incorporate recovery principles by considering the individual’s and their family’s strengths, barriers, and needs
Document treatment goals/objectives/discharge plans in clear language and include clients’ own words
Formulate measurable, achievable goals within the scope of treatment in collaboration with the client
Describe clinical modalities/interventions that are responsible and in line with applicable protocols
Reassess treatment effectiveness and adapt the treatment plan as needed
Maintain continuity among clinical teams by documenting and communicating actions, irregularities, and continuing needs
Occasionally lift and/or move up to 25 pounds
Other duties as assigned
Qualifications
Must have one of the following: CAT/CAS, LAC/LCSW/LPC, or a Masters in Social Work/Behavioral Health
Licensure/certification as required by the role
Experience: Addiction counseling, 2 years (Preferred)
