
Lead Care Manager

Job Summary
EA Family Services is hiring a full‑time Lead Care Manager for the Bakersfield area. The role involves working with a dynamic team to provide care coordination and case management for Medi‑Cal enrollees. The successful candidate will have a respectful, objective, friendly, and outgoing attitude, and be comfortable working with people.
Responsibilities
Coordinate with individuals and/or entities to ensure a seamless experience for the member and prevent duplication of services.
Engage eligible members.
Oversee provision of ECM and CS services, including completion of the assessment, development of a care plan, and implementation of the care plan.
Complete referrals on behalf of members.
Offer services where the member lives, seeks care, or finds most easily accessible and within managed care plan guidelines.
Connect the member to other social services and supports they may need, including transportation and community supports.
Advocate on behalf of members with healthcare professionals.
Use motivational interviewing, trauma‑informed care, and harm‑reduction approaches.
Coordinate with hospital staff on discharge plans.
Accompany members to office visits, as needed and according to managed care plan guidelines.
Monitor treatment adherence, including medication.
Provide health promotion and self‑management training.

Clinical protocols and evidence‑based practice guidelines
Population of focus for specific health needs, cultural awareness, and sensitivity

Qualifications
Education: Bachelor’s degree in a social services field (Required)
Experience: 1 year of case management (Preferred)
Language skills: Bilingual in English and Spanish (Required)
Certifications: CPR Certification (Required)
Security: Must pass DOJ/FBI/CAIC clearances (Required)
Other: Ability to commute or relocate; willing to travel up to 25% (Preferred)
