Community Resources Navigator

Martha's Vineyard Hospital is seeking a Patient Services Coordinator to assist patients with transportation and connect them to community resources. Responsibilities include collaborating with case managers for discharge planning and educating patients and families about available services. The ideal candidate should have knowledge of medical terminology and community resources, and possess excellent communication skills. This role is expected to be onsite, with scheduled hours of 37.5 per week.
