Qualifications
Current Texas Licensed Vocational Nurse (LPN) license required
Minimum of 2-3 years as a Licensed Vocational Nurse (LPN)
Current CPR certification required
Dependable transportation, driver’s license, and car insurance
Previous Hospice experience preferred
Job Duties
Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
Communicates with attending physicians, other hospice team members, and agencies as needed to coordinate optimal care and use of community resources for the patient/family.
Documents all services provided and submits them on appropriate forms.
Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
Reports changes in patient condition and needs to RN.
Great time management skills.
