We are seeking a compassionate
Adult/Geriatric Nurse Practitioner
to join our growing home-based care team in the San Fernando Valley & Antelope Valley area. This is a meaningful field role focused on caring for patients where they are most comfortable — at home — during some of the most important moments in their recovery.
You will manage an assigned panel of patients receiving
acute and transitional care in the home , including individuals recently discharged from the hospital, emergency department, or skilled nursing facilities, as well as medically complex, homebound patients with lower acuity needs.

Responsibilities
Conduct in-home health assessments and focused physical exams
Manage acute and post-acute conditions for patients transitioning from hospital, ER, or nursing facilities
Develop, implement, and document individualized care plans
Order, interpret, and evaluate diagnostic tests
Prescribe medications and treatments within scope of practice
Collaborate closely with physicians, care managers, and interdisciplinary team members
Educate patients and families to support recovery, safety, and long-term health at home
Maintain timely, accurate clinical documentation

Schedule: Full-time, Monday–Friday
Salary Range: $119,312.60 – $178,967.80 annually
Bonus eligible based on individual and company performance
Qualifications
Master’s degree in Nursing (MSN)
Adult/Geriatric Nurse Practitioner certification
Minimum 1 year of Nurse Practitioner clinical experience (home-based or transitional care experience a plus)
Active, unrestricted RN and NP license in the applicable state
Current TB test or willingness to obtain prior to start
Compensation
$119,313.00 to $178,968.00
