
PT Physical Therapist FT

Key Responsibilities
Implement physician-approved care plans
– Carry out physical therapy treatment plans established by the supervising Physical Therapist and physician, delivering skilled interventions in the patient's home.
Deliver direct therapeutic treatment
– Administer therapeutic exercises, manual therapy, massage, gait and balance training, and physical modalities (heat, cold, ultrasound, electrical stimulation) to relieve pain, restore function, and improve mobility.
Provide assistive device training
– Instruct patients on the safe use of walkers, crutches, canes, wheelchairs, braces, and prosthetics to support independent functioning at home.
Educate patients and families
– Teach home exercise programs, proper body mechanics, and medical equipment safety to patients and caregivers to promote safe, confident recovery between visits.
Monitor and document patient progress
– Observe patients before, during, and after each session; record clinical findings; and submit timely progress notes to inform the supervising PT and care team.
Communicate changes in patient condition
– Report any changes in a patient's status or response to treatment to the Physical Therapist promptly, recommending plan‑of‑care modifications as needed.
Coordinate interdisciplinary care
– Participate in care conferences, collaborate with agency personnel (nursing, OT, social work), and keep the team informed of patient progress to ensure seamless, holistic care.
Qualifications
Be able to implement care plans and communicate any findings or changes in regards to patients back to the Physical Therapist and physician.
Ability to educate patients and their families in basic home safety and medical equipment safety.
Required Certifications and Licensures
Associate's degree from an accredited school of Physical Therapy.
Have a valid license, registration, or permit to practice in state of agency operation.
Must possess and maintain valid CPR certification while employed in a clinical role.
Must be a licensed driver who can travel to all business locations.
