Job Description

We are seeking a

Physical Therapist

to deliver high-quality in-house therapy services to adults with a range of diagnoses and disabilities, with a special focus on supporting aging in place. In this role, you will directly influence residents\' quality of life by providing person-centered interventions that promote mobility, independence, and overall well-being. Primary Responsibilities
Conducts screenings of new community residents and those referred by nursing to determine the need for intervention

Evaluates residents to obtain data necessary for treatment, planning and implementation

Interprets and communicates evaluation findings to residents, family, and other team members

Records treatment times and documentation by following federal and state laws, rules, and regulations

Demonstrates capacity to understand insurance reimbursement and billing while coordinating care

Provides treatment in accordance with established standards of practice, department procedures and productivity standards while adapting treatment for individual needs

Monitors resident response to intervention, modifying treatment as indicated to attain goals in accordance with physician orders

Recommends and educates residents, family, and community staff in the use of adaptive equipment and assistive devices

Consistently demonstrates sound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programs including excellent communication with community staff

Maintains safety standards including safety stands including compliance with infection control, universal precautions and OSHA stands for healthcare professionals

Qualifications
Graduate of an accredited therapy program - Physical Therapist (PT)

Current state licensure

Excellent verbal, written and interpersonal communication skills

Results-oriented with strong critical thinking and problem-solving skills
