Position Summary
The Patient Care Assistant is an unlicensed staff member who is accountable to/completes delegated tasks from the RN in a caring and culturally sensitive manner with excellent customer service at all times. Demonstrates competence in critical thinking, interpersonal relationships, and technical skills related to delegated tasks. Provides basic nursing skills including but not limited to vital signs and pulse oximetry, finger stick blood glucose, ADLs, recording intake and output, collecting specimens, applying/removing compression hose, SCDs, emptying drains, urinary catheters, NG tubes, and ostomies, and additional skills to meet unit needs. Documents all tasks in O2, I&O, percentage of meals eaten, etc. Maintains a neat, clutter-free, clean patient room and work environment. Maintains a safe patient environment to include high fall risk interventions as appropriate.

Responsibilities and Essential Job Functions

Provides basic nursing skills such as vital signs, pulse oximetry, finger stick blood glucose, ADLs, recording intake and output, collecting urine, stool, sputum specimens, applying/removing compression hose, SCDs, and other required tasks.

Documents all tasks in O2, I&O, percentage of meals eaten, etc., and keeps the patient room and work environment neat and clean.

Maintains a safe environment for high fall risk patients: conducts hourly rounds, responds promptly to call lights, observes hand hygiene, follows standard precautions and isolation principles.

Provides care for patients in restraints, ROM and fluids/toileting, reorienting confused patients, and reinforces patient/family education regarding fall and safety precautions.

Notifies the RN promptly of any changes in vital signs, mental status, skin integrity, pain reports, or other patient needs.

Performs professional, clinical, and/or technical competencies of the assigned unit or department and may complete other duties as assigned.

Skills and duties may vary by department or unit; other duties may be assigned.

Required Education and Experience

High school graduate or GED, or currently enrolled in a high school or GED program.

And one of the following: hold a CNA license or have completed the Gold Standard Intern Program; have documentation of fundamentals of nursing courses; have at least 18 months of relevant patient care experience.
