Pay Ranges: $60.00 - $70.00 per hour (Based on Experience)

Bonus for Full-Time Position: $4,000 Hiring & Retention Bonus (Limited Time Offer)

We are seeking a Physical Therapist to provide high-quality rehabilitative care to residents. This role focuses on improving mobility, strength, balance, and functional independence to enhance residents’ safety and overall quality of life. Reporting: This position collaborates closely with the rehabilitation team, interdisciplinary staff, and physicians to support comprehensive care planning.

Role Overview

Evaluate residents and develop individualized physical therapy treatment plans

Implement evidence-based interventions to improve functional outcomes

Document resident progress accurately and in compliance with regulatory requirements

Collaborate with interdisciplinary teams and physicians

Supervise Physical Therapist Assistants and therapy aides, as applicable

Monitor clinical outcomes and contribute to departmental performance

What We’re Looking For

Active California Physical Therapist license in good standing

Bachelor’s, Master’s, or Doctorate degree in Physical Therapy

New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferred

Knowledge of PDPM, Medicare, and skilled nursing facility regulations

Strong leadership, communication, and organizational skills

Commitment to resident-centered, high-quality care
