Responsibilities and Duties
Implement plan of care in consultation with physical therapist and other careteam members.
Communicate findings or changes in regard to patients back to the physical therapist and other careteam members.
Educate patients and their families in basic home safety and medical equipment safety.
Solution driven, selfmotivated, and inspiring to patients to reach their goals.
Work independently without direct oversight.
Able to discern when to call for support and communicate challenges.
Familiar and comfortable with technology.
Qualifications and Skills
Associate Degree in Physical Therapy
Current Certification in BSL/CPR
Home Health Experience
Valid Texas Driver's License and Automobile Insurance
Knowledge of WellSky EMR software is a plus
