Overview
NOVA Patient Care is looking for a compassionate, detail-driven LPN to join our Virginia-based clinic team in a dual-function role that blends hands-on clinical support with meaningful care coordination.

Role: LPN – Ancillary Clinical Support & Care Coordination (Full-Time)

Experience: Minimum 2 years in a clinical setting
What You''ll Do
As ancillary support staff in our clinics, you''ll be the backbone of daily patient flow — rooming patients, recording vitals, maintaining EHR records, supporting providers during exams, collecting lab specimens, and ensuring a clean, compliant, and efficient clinical environment.
On the care coordination side, you''ll manage our Chronic Care Management (CCM) program — enrolling eligible patients, building individualized care plans, conducting monthly check-ins, tracking CCM minutes for billing and supporting Remote Patient Monitoring (RPM) activities.
This is not a routine nursing role. It''s a chance to own your patient relationships and be a central point of continuity in their care journey.

Requirements
Active LPN license in Virginia (required)
Experience in primary care, family medicine, or outpatient settings
Knowledge of CCM programs, CMS guidelines, or care coordination workflows
Proficiency with EHR systems (Epic, Athena, ECW, or similar)
Ancillary clinical support certification (CMA, RMA, or equivalent) is a plus
