Job Description Summary
The Clinical Care Coordinator provides administrative support to the Clinical Care team by managing the electronic medical records system. The Clinical Care Coordinator also supports administrative functions of the back office to include, but not limited to: billing, payroll, human resources and intake. The Clinical Care Coordinator offers training and ongoing support to new Clinical Care Coordinators.
Essential Job Functions/Responsibilities
Accurately enters a variety of data into various information systems
Executes workflow functions in electronic medical record system
Verifies accuracy of data entered through collaboration with Clinical Manager(s) and/or Clinical Director
Manages incoming, outgoing and interoffice mail
Provides customer support to patients and colleagues via phone, email and fax
Alerts appropriate management team members regarding potential compliance issues
Maintains confidentiality of PHI and organization information at all times
Order DME and alerts DME supplier when equipment is no longer in use

Participates in and scribes IDG meetings
Performs other specific projects relating to office procedures
Provides training and ongoing support to new Clinical Care Coordinators within the region
Position Qualifications
High School Diploma (College Degree Preferred)
At least two (2) years health care experience, preferably in hospice
Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
HCHB/Pointcare knowledge is preferred
Proficient in MS Office and Google Applications
