Cambridge Health Alliance (CHA) is a leader in Primary Care, serving a diverse population across the Boston metro-north region. CHA operates 13 primary care centers, 3 teen health centers, telehealth services, and in-home programs. CHA is a teaching hospital with residency programs in internal medicine and family medicine.
Job Summary

Under the supervision of the Nurse Manager, the Remote Nurse Advice RN handles telephonic and electronic patient portal messages to triage symptom-based concerns. The nurse assesses patient needs and clinical symptoms, follows evidence-based protocols embedded in the EMR, and provides appropriate clinical advice. The nurse schedules appointments with primary providers, specialists, and community resources as needed, and provides education and counseling to patients of all ages, including prenatal patients. Documentation is completed in Epic EMR as appropriate. This position is primarily remote with a period of on-site training required.
Responsibilities

Handle telephonic and electronic patient portal messages to triage symptom-based concerns.
Assess patient needs and clinical symptoms and apply evidence-based protocols in the EMR.
Provide appropriate clinical advice and education to patients.
Schedule appointments with primary providers, specialists, and community resources as needed.
Educate and counsel patients on a variety of health topics; document in Epic EMR.
Qualifications

Education/Training: Graduate of an accredited school of nursing. BSN preferred.
Licensure: Current or conditional MA RN Licensure required.
Work Experience: Minimum 2 years of current primary care telephone triage experience; family medicine experience preferred.
Linguistic Capabilities: Ability to speak a second language preferred.
