You will guide families through care decisions, while providing comfort, dignity, and compassionate support during life’s most tender moments. You will:* Complete comprehensive assessments of patients and families to determine hospice needs* Develop and implement individualized care plans, revising as needed* Provide skilled nursing care, administer medications, and manage symptoms* Educate patients and families on care techniques, medications, and comfort measures* Maintain accurate clinical documentation and communicate changes to physicians* Participate in interdisciplinary team meetings and quality improvement initiatives* Provide emotional support and counseling to patients and families* Supervise hospice aides and delegate responsibilities appropriately***Our Commitment to You**** Flexible schedules to fit your life and support balance* Ongoing professional development and training* A truly collaborative team that celebrates your success***Requirements**** Currently licensed as a Registered Nurse (RN) in the State of Arizona* Minimum of 2 years of nursing experience, hospice or home health experience preferred* Homecare Homebase (HCHB) experience preferred, not required* Valid driver’s license and reliable, insured vehicle* Strong clinical skills and a passion for patient-centered careThe employer for this position is stated in the job posting.
